How to join the Cavity-Free Club:

1) You will get a ticket from your hygeinist or Dr. Pat's assistant as long as the following criteria ar met:
Always SMILE!
Come in for your regularly scheduled recares (check-ups)
Follow brushing instructions
Be cavity-free at wellcare visit
If you have a cavity or cavities, you will receive a ticket when the filling(s) have been completed

2) Turn in your ticket at the front desk to be entered into our quarterly drawing. 4 lucky winners will get a gift certificate for $25.00 to Amato's Toy Store!
